In the end, after Ichiro’s murder, Chae-ok goes missing. Tae-sang searches for her despite Maeda’s news of her death, without proof.

Tae-sang opposes Maeda, who secretly controls Ongseong Hospital’s funding. This puts Tae-sang in danger as he falls out of Maeda’s favor. Witnessing the Japanese army’s cruelty, Tae-sang decides to destroy the hospital.

With rebel help, he plans to use dynamite. However, their initial plan fails, and they strike a deal to target the hospital. Tae-sang sneaks in armed with explosives to find Chae-ok.

Meanwhile, Chae-ok is given to Kato by Maeda. Kato tries using her and her father against Seishin. Eventually, Tae-sang meets Jung-won, who sacrifices himself to destroy the hospital and asks Tae-sang to protect Chae-ok.

Chae-ok escapes with Tae-sang’s help but is distraught by her father’s choice. The hospital explodes, and amidst the chaos, Chae-ok is seemingly dead, devastating Tae-sang.

He refuses to believe she’s gone and is relieved when she reappears. Despite arranging an escape for her, Tae-sang decides to stay by Chae-ok’s side.

They are pursued by Yakuza hired by Maeda, leading to a fatal confrontation where Chae-ok sacrifices herself to save Tae-sang. Tae-sang struggles with her loss but later discovers the possibility of Chae-ok’s resurrection via a parasitic transfer orchestrated by Seishin. Furthermore, Myeong-ja’s disappearance led to Tae-sang’s involvement with the hospital.

Ishikawa, desperate for Myeong-ja’s return, faces her after she turns into a killer influenced by Najin. Despite Ishikawa’s anger, he abandons Myeong-ja and allows her to be taken back to the hospital for experimentation.

In the chaos, Kato uses Myeong-ja to birth a child infused with Najin, sacrificing her life for his experiments, potentially creating a monster under his control.
